    Welcome to City Hospital campus Diabetes Service For Adults With Diabetes    
    We aim to provide comprehensive care for all people over 16 years old with diabetes who have been referred to us. The following pages contain information about the diabetes service, which we hope you will find helpful. If you would like to use our services but are not under the care of one of the diabetes consultants, please speak to your GP who may be able to refer you. People with Type 1 diabetes are usually seen in hospital clinics. People with Type 2 diabetes are usually treated and monitored by their GPs but may need to be referred to hospital clinics if particular problems arise.

All Outpatient Diabetes care apart from antenatal is held at Dundee House Diabetes Unit.

The hospital is situated a few miles north of Nottingham city centre, by the ring road, and between Hucknall Road, and Edwards Lane.

Dundee House is situated within the City Hospital grounds it is best to approach from Edward's Lane, entrance 5 of City Hospital campus. It is opposite the south corridor wards and the yellow car park. There is limited disabled parking at the centre at the back, the nearest car park is in yellow car park the tarmac area nearest to Dundee House,if full there is also purple car park (entrance 4 of the hospital). Buses 88,89 and 90 go along Edwards lane.

    1. Outpatient Clinics    
 
Check up     A General Diabetic Clinics    
      Everybody should have an annual review at least once a year. A General Clinic is held weekly at Dundee House. Dr's Game, Page and Chokkalingham together with Diabetes Nurses and Dietitian run this clinic. Once all the issues related to your diabetes or general health are dealt with, then you will be discharged back to your GP.    
 
      B Clinics For Special Groups    
        i Nephropathy Clinic
This is held weekly at Dundee House. It is run by Dr Sturrock and Dr Archer together with a Diabetes Nurse and Dietitian. Kidney changes are picked up either from a urine test (proteinuria) or a blood test (raised creatinine). Patients who have changes in their kidneys due to diabetes or blood pressure are seen in this clinic if they would benefit from concerted attempts to control their blood pressure. There is a close liaison with the Renal Unit.

This is a twice-weekly clinic held at Dundee House. Both Professor Jeffcoate and Dr Game run this clinic together with Podiatry, Diabetes Nurses, Plaster Room Nurses and an Orthotist. Vascular surgeons are present on most weeks. Patients with foot ulcers are seen here.

   
        iii Cardiac Risk Clinic
This is held weekly at Dundee House and is run by Dr Chokkalingham and Dr Haq together with a Diabetes Nurse and a Dietitian. Patients with known hardening of the arteries, especially following a heart attack, or those who are at risk of it are seen in this clinic. Management includes trying to reduce the risk of further problems by modifying risk factors for hardening of the arteries including high blood pressure, raised cholesterol, diabetes control that could be better or smoking.

   
Antenatal Clinic       iv Combined Antenatal Clinic
Women with diabetes or who develop diabetes in pregnancy are seen in this clinic during their pregnancy. Dr R Page and Dr A Archer run it together with the Obstetricians (doctors who specialise in pregnancy) midwives and Diabetes nurses and Dietitian. It is held weekly in the Maternity Unit.

   
        v Cystic Fibrosis Clinic
This is held monthly. Dr Archer and the Respiratory Physicians together with a Diabetes Nurse and CF Nurses will see patients with Cystic Fibrosis who have developed diabetes. It is held at Dundee House.

   
Evening Clinic       vi Evening Clinic
This is a clinic for patients with Type 1 Diabetes. It is run by Dr Jeffcoate together with one of the Diabetes nurses. It is held at Dundee House fortnightly.


   
        vii Under 25 Clinic
This is held fortnightly. Dr Clayton runs this clinic together with a Diabetes nurse and Dietitian.

   
        viii Transition Clinic
This clinic is run by Dr Drew (paediatric diabetes doctor) and Dr Clayton (adult diabetes doctor) with both the paediatric and adult diabetes nurses.

This clinic is designed for those young people preparing to transfer from the paediatric service to the adult service and is held every 4 months.

   
        ix Type 1 structured education programme
EDWARD (Education for Diabetes Without A restrictive Diet). This is a course for people with Type 1 diabetes to help you learn how to manage and feel in control of your own diabetes. EDWARD is held one day a week (either Wednesday or Thursday) for four weeks. There are follow up sessions held 3 monthly.

   
        x Diabetes Support Clinic
Dr Archer runs a weekly Diabetes Support Clinic on a Thursday afternoon. The clinic is for people with life concerns that are affecting their diabetes care or vice-versa. Referrals are made within the diabetes department. The service is totally confidential.

   
    Note: We are a teaching hospital and therefore in both Doctors and Nurse/Dietitian clinic there may be other health care professionals observing.    
    2. Individual Appointments with Nurses and Dietitian
You may receive an appointment to see the diabetes specialist nurses and Dietitians before you have an appointment to see the doctor. You will not see a doctor at this appointment but it will allow you the opportunity to learn more about diabetes and to discuss any aspect of your diabetes care. You will receive individual advice in an informal and relaxed setting. It will be useful if you can bring with you a list of any medication you are taking.

Nurse and Dietitian clinics are held at Dundee House for new and follow up patients Monday to Friday mainly in the morning.

    3. Support Service
There is a telephone answering service provided for patients whose diabetes is under the care of The City Hospital. The diabetes nurses can be contacted for advice on diabetes and related problems, 9am - 4.30pm Monday - Friday on 0115 9627621 (excluding Bank Holidays). Calls are answered as soon as possible.

Urgent Messages - NB You are advised not to leave urgent messages or queries on the answering machine. In an Emergency please contact your GP.
   
Leaflets   4. Education
Advice leaflets on many aspects of diabetes are available for short-term loan. A full range of educational programmes is available for patients on diet, tablets or insulin. Please ask for details.
   
Pregnancy And Diabetes   5. Pregnancy and Diabetes
Diabetic control is very important before and during pregnancy: getting pregnant when your control of diabetes is poor can be dangerous for your baby's health. Any woman considering pregnancy should contact the diabetes specialist nurses. We provide a joint diabetes/antenatal service with the maternity department and offer pre-pregnancy advice sessions on an individual basis or as a course in groups. Folic acid 5mg supplements should be taken before every pregnancy if possible (available in this dose on prescription). It is also important to stop smoking if you are planning a pregnancy.

Research is of course very important to help us understand more about diabetes and to improve treatments. There are always a variety of research projects in progress. If you would like to take part in a project, please let us know and we will find out if a suitable project for you is underway.
